Endoscopic Endonasal Approach for Pituitary Adenoma Resection with Comorbid Fibrous Dysplasia

We present an endoscopic endonasal approach for a pituitary adenoma in a patient with polyostotic fibrous dysplasia. The surgical techniques employed were challenging due to fibrous dysplasia extensively involving the sinonasal cavity and the skull base. Extensive drilling was used to visualize the posterior nasal cavity. Bilateral sphenoid sinuses were replaced by fibrotic tissue, thus, drilling the rostrum and carefully removing fibrotic tissue over the bone overlying the sella allowed access to the adenoma.
