Lateral Orbitotomy in a Pediatric Patient For a Complex CSF Leak Via Superior Orbital Fissure And Foramen Rotundum Following Delayed Failure of Expanded Endoscopic Endonasal Transpterygoid Repair
A 3-year-old patient presented with recurrent chest infections and complex CSF leak from the right superior orbital fissure and foramen rotundum. Patient had recurrence of CSF leak 14 months following an initial expanded endoscopic endonasal transpterygoid repair. A lateral orbitotomy with maxillary nerve sacrifice followed by multilayered repair was undertaken leading to successful repair and resolution of chest symptoms with the latter likely due to silent CSF aspiration.
