Onodi Cell Dehiscence: A Rare Cause of Ophthalmic Artery Injury in Endonasal Surgery

A 37-year-old male underwent trans-sphenoidal surgery for a pituitary macroadenoma. While preoperative MR angiography was normal, CT revealed bilateral Onodi cells. During the procedure, an arterial bleed occurred near the right opticocarotid recess, controlled with compression. Postoperative imaging showed a right ophthalmic artery pseudoaneurysm, treated with coil embolization. The distal artery was perfused via external carotid perforators, and the patient’s vision was preserved. This case highlights the risk of vascular injury associated with anatomical variants.
