A 59-year-old male presented with proptosis and frontal bossing together with nasal obstruction and anosmia. CT, MRI nasal, and PNS showed a huge recurrent sinonasal mass based on the frontal sinus bilaterally and extending into the ethmoid sinus. The video presents Draf III procedure with the drill-out technique with the following steps: superior septectomy, drilling of the frontal peak, identification of the frontal sinus, excision of the mass, clearance of the frontal recess, and ethmoid sinus.
