Surgical Treatment of Suprasellar Dermoid Cyst via Endoscopic Endonasal Approach

We present the case of a 35-year-old patient demonstrating diplopia secondary to complete right third nerve palsy. An MRI revealed a dermoid cyst in the right suprasellar cistern, deforming the right cerebral peduncle. Near-total resection of the lesion was accomplished via an endoscopic endonasal approach. An intraoperative decision was made to split the pituitary gland to access the lesion, which was well tolerated without postoperative hormonal dysfunction. The patient’s third nerve palsy also improved postoperatively.
