A 58-year-old woman with bilateral trigeminal neuralgia underwent bilateral microvascular decompression surgery in two consecutive craniotomies. The procedure involved separating blood vessels and placing insulating material. The right surgery was extensive due to a prominent suprameatal tubercle, which had to be drilled. Minor postoperative side effects were reported. The bilateral pain was relieved, and the patient remained pain-free at follow-up.
