A 74-year-old, left-handed male presented with 1-year history of fatigue and new left ptosis and was incidentally found to have a right medial temporal glioblastoma. Given that a portion of his speech mapped to the right side, he underwent a right lateral transorbital approach for resection of the medial temporal glioblastoma. We present the operative video detailing the approach and operative technique.
