Retrograde Intubation eines primär nicht intubierbaren Patienten

Zusammenfassung

Dargestellt wird die retrograde Intubation eines respiratorisch insuffizienten Patienten, bei dem die orotracheale Intubation primär nicht gelang. Nach Eindringen einer Führungssonde - über eine Punktion des Ligamentum cricothyroideum -, die nach oralwärts ausgeleitet wird, kann komplikationslos in kurzer Zeit (ca. 30 - 45 Sekunden) ein Orotrachealtubus plaziert werden. Bei Nichtgelingen der oro-/nasotrachealen Intubation sollte, unter Berücksichtigung der möglichen Komplikationen, diese Methode als praktikabel und im Vergleich zu anderen Methoden, wie der Koniotomie, als weniger invasiv berücksichtigt werden.

We present the case of a young patient experiencing respiratory insufficiency, in which orotracheal intubation was unsuccessful. After puncturing the laryngeal ligament with a i. v. cannula, we inserted a thin catheter through the needle and guided it orally through the mouth. The patient was then intubated orotracheally, using the catheter as a guide. Beginning with the laryngeal puncture we need 30 to 45 seconds for the procedure. In spite of the possible complications of retrograde intubation, it should be considered as a feasible alternative in case of unsuccessful orotracheal intubation.
