Zusammenfassung

Sucht im Alter ist ein bislang zu wenig beachtetes Problem. Vorurteile in Bezug auf die Lebenssituation älterer Menschen und Fehleinschätzung der therapeutischen Möglichkeiten führten zu einem therapeutischen Nihilismus. Die Diagnose ist im Alter oft schwierig wegen der differenzialdiagnostischen Abgrenzung zu alterstypischen Einschränkungen. Late-onset-Trinker mit Beginn des Alkoholabusus im höheren Lebensalter haben eine günstige Prognose. Aufgrund physiologischer Veränderungen im Alter treten Intoxikationen und Toleranzentwicklung schon bei geringen Trinkmengen auf, praktisch alle Alkoholfolgekrankheiten kumulieren im Alter. Die sozialen Auswirkungen der Suchterkrankung führen zur Vereinsamung und zur Beeinträchtigung der Lebensqualität. Die Rehabilitation suchtkranker älterer Patienten durch ambulante und stationäre Behandlungsangebote ist erfolgversprechend. In diesem Artikel wird ein Überblick über die Literatur gegeben und über eigene Erfahrungen berichtet.

Rehabilitation of Elderly Patients with Substance Use Disorders

Addiction in the elderly is an often neglected problem. The prejudices concerning the situation of older people and the missing knowledge about therapeutic options led to a nihilistic attitude. The diagnosis of an addictive disorder in the elderly is often difficult to be made because of the similarity to deficits typical for this age. In many cases the problem gains clinical attention when medical complications occur. Late onset alcoholics who start drinking at a later age have a better prognosis. Because of alterations in the physiology of older people intoxications and the development of dependence occur with the consumption of lower amounts of alcohol. All medical consequences of alcoholism cumulate in the elderly. Social sequelae of the addictive disorder are isolation and a low quality of life. Rehabilitation with out- and inpatient treatment programs are successful concerning the physical and psychosocial conditions. This article gives a survey of the literature on this topic and a report about own experiences.
