Granulomatöse Amyloidose - ein charakteristisches Substrat der Transthyretin-Amyloidose beim Karpaltunnelsyndrom?

Zusammenfassung

Amyloidablagerungen im synovialen Gewebe sind meist auf ein bisher nicht klassifiziertes seniles Amyloid des Bindegewebes zurückzuführen - histologisch zeichnen sie sich durch kleine Herde eines kongophilen Substrats ohne zelluläre Reaktion aus. In der kurzen Kasuistik über zwei Patienten mit seniler Transthyretin-Amyloidose und Karpaltunnelsyndrom werden granulomatöse zelluläre Reaktionen um die Amyloidablagerungen beschrieben. Es wird vermutet, dass dieses granulomatöse Erscheinungsbild um Amyloidablagerungen im synovialen Gewebe ein Indikator für die Transthyretin-Amyloidose sein kann - eine immunhistologische Bestätigung durch Bestimmung des Amyloidtyps ist allerdings notwendig.

Granulomatous Amyloidosis - A Characteristic Substrate of Transthyretin Amyloidosis in Carpal Tunnel Syndrome

Amyloid deposits in the synovial tissue are usually due to a hitherto unclassified senile amyloid of the connective tissue - histologically they are characterised as foci of a congophilic substrate without cellular reaction. In this report two cases of senile transthyretin amyloidosis in patients with carpal tunnel syndrome, amyloid deposits with a surrounding granulomatous cellular reaction are described. It is assumed that this granulomatous appearance of the amyloid deposits in the synovial tissue may be an indicator of this type of amyloidosis - nevertheless, the type of amyloid should be supported by immunohistology.
