Prädiktoren des Behandlungserfolges in der stationären psychosomatischen Rehabilitation muskuloskelettaler Erkrankungen

Zusammenfassung

Ziel der vorliegenden Studie war die Identifikation von Ausgangs- und Veränderungswerten, die den kurz- (Ende der Therapie) und langfristigen (drei Jahre nach dem Aufenthalt) Therapieerfolg in der psychosomatischen Rehabilitation muskuloskelettaler Erkrankungen prognostizieren. In der Analyse der Ausgangswerte zeigte sich, dass der kurz- und langfristige Behandlungserfolg vor allem durch einen nicht vorhandenen Rentenwunsch, eine geringere Anzahl ärztlicher Behandlungen sowie eine niedrigere Arbeitsunfähigkeitszeit prognostiziert wurde. Die diskriminanzanalytische Untersuchung der Veränderungswerte ergab, dass für den kurzfristigen Erfolg vor allem die Verbesserung der körperlichen Funktionsfähigkeit und des psychischen Wohlbefindens sowie Veränderungen in der Selbstwertregulation Relevanz besaßen; den langfristigen Erfolg prognostizierten die Verbesserung des psychischen Befindens sowie die Verständigung zwischen Arzt und Patient und eine Abnahme der Depressivität im Therapieverlauf.

Predictors of Therapeutic Success in Inpatient Psychosomatic Rehabilitation of Musculo-Skeletal Diseases

The aim of the present study was to identify pre-program factors and treatment-related changes predicting short-term (end of treatment) and long-term (three years after therapy) treatment success in inpatient psychosomatic rehabilitation of musculo-skeletal diseases. Discriminant function analysis of pre-program factors above all revealed a positive influence of a non-existing wish for social compensation, a smaller amount of medical treatment and lower sick leave for short- and long-term therapeutic outcome. Analysis of treatment related changes showed, that an improved mobility, well-being and an ameliovated self-esteem had prognostic value for short-term success. Long-term success was predicted by improved well-being, an improvement of understanding between physician and patient and by decreased depressive symptoms in the course of treatment.
