Die Berner Psychotherapie-Tagesklinik: Evaluation und Einordnung in die psychiatrische Versorgungskette

Zusammenfassung

In der kognitiv-verhaltenstherapeutisch orientierten Berner Psychotherapie-Tagesklinik werden Patienten mit Depressionen, Essstörungen, Angst- und Zwangsstörungen sowie Persönlichkeitsstörungen für die Dauer von 12 - 20 Wochen behandelt. Das Konzept wird im Kontext ähnlicher Institutionen beschrieben. Zur Evaluation der Veränderung während des Therapiezeitraumes werden der BDI, der SCL-90-R und der VLB von 89 Patienten herangezogen. Die Patienten zeigen ein hohes Symptomniveau bei Beginn der Behandlung und markante Verbesserungen während des Therapiezeitraumes. Patienten mit Persönlichkeitsstörungen zeigen geringere Verbesserungen im Bereich der sozialen Anpassung als Patienten ohne Persönlichkeitsstörung. Die Ergebnisse des SCL-90-R bezüglich Symptomausgangsniveau und Veränderung während des Therapiezeitraumes deuten darauf hin, dass in der Berner Psychotherapie-Tagesklinik ähnlich stark beeinträchtigte Patienten behandelt werden wie sonst in stationären oder spezialisierten tagesklinischen Psychotherapieinstitutionen. Es werden auch vergleichbare Effekte erzielt wie in diesen Einrichtungen. Es wird argumentiert, dass das hier vorgestellte Behandlungskonzept besonders für die gemeindenahe Versorgung von schwerer gestörten nichtpsychotischen Patienten geeignet erscheint und auch in Regionen mit geringer Bevölkerungsdichte anwendbar ist.

Abstract

The Bern Day Treatment Programme offers cognitive-behavioural treatment to patients suffering from depression, eating disorders, anxiety disorders, and personality disorders. Treatment duration ranges from 12 to 20 weeks. Its concept is described in comparison to other similar institutions. To evaluate the programme the Beck Depression Inventory (BDI), the Symptom Check List (SCL-90-R), and the Change in Life Areas (VLB) were administered to 89 patients at admission and discharge. Their analyses revealed high symptom levels at admission and a significant improvement during treatment. Patients with personality disorders show less improvement in the VLB than patients without personality disorders. The results of the SCL-90-R indicate that the symptom levels at admission and the symptom reductions during treatment are similar to those of patients treated in inpatient units or in diagnostically specialised day treatment programmes. The Bern Day Treatment Programme seems to be feasible for highly impacted patients with non-psychotic disorders and might be a useful model for areas with low population density.
