Niederfrequente Ultraschalltherapie bei muskuloskelettalen Beschwerden

Zusammenfassung

Fragestellung: Es wird der Frage nachgegangen, ob niederfrequenter Ultraschall von 30 bis 120 kHz bei bestimmten Indikationen, wie Myalgien und lumbalen Rückenschmerzen, therapeutische Vorteile gegenüber dem bekannten hochfrequenten Ultraschall von 1 bis 3 MHz bietet. Methode: Die physikalischen Grundlagen des niederfrequenten Ultraschalls werden dargestellt und die Eignung als Therapiemittel bewertet. In den Datenbanken MEDLINE, EMBASE und SCISEARCH wurde von 1990 bis 2000 nach relevanter Literatur zur niederfrequenten Ultraschalltherapie gesucht. Ergebnisse: Insgesamt wurden 11 Arbeiten zum Thema gefunden und 8 Arbeiten in die Auswertung eingeschlossen. Die Ergebnisse zeigen, dass niederfrequenter Ultraschall eine analgetische Sofortwirkung hat, zu einem frühen Wirkungseinsatz bei serieller Anwendung führt und eine große Langzeitwirkung aufweist. Behandlungszeiten werden verkürzt und die Patientencompliance verbessert. Mögliche Gefährdungen für den Patienten sind geringer als bei hochfrequentem Ultraschall. Schlussfolgerung: Niederfrequenter Ultraschall eröffnet neue Möglichkeiten für die Behandlung muskuloskelettaler Schmerzen. Die Anzahl der Publikationen ist noch sehr klein und die Fragen der optimalen Ultraschallfrequenz und Dosierung sind offen. Weitere Studien für einen wissenschaftlich gesicherten Wirkungsnachweis sind notwendig.

Abstract

Purpose: The question is pursued, whether low-frequency ultrasound of 30 to 120 kHz offers therapeutical advantages with specific indications, as myodynia and low back pain, compared with the well-known high-frequency ultrasound of 1 to 3 MHz. Methods: The physical basics of low-frequency ultrasound are presented and the suitability is assessed for ultrasonic therapy. A literature-search for relevant publications of low-frequency ultrasonic therapy was done by using the databases MEDLINE, EMBASE and SCISEARCH from 1990 to 2000. Results: In total there were found 11 publications with the subject and 8 papers are enclosed to the analysis. The results show, that low-frequency ultrasound gives an immediate analgetic effect, leads to an early beginning of activity at serial application and a great long-term action. Time of treatment can be reduced and the patient's compliance improved. Possible risks for the patient are smaller than with high-frequency ultrasonic treatment. Conclusions: Low-frequency ultrasound sets up a new way for the treatment of musculoskeletal pain. The number of publications is still very small and the questions of the optimal ultrasonic frequency and dosage remain open. Further studies will be needed for the scientific-based evidence of treatment.

3 Gegenwärtig entsteht die VDI-Richtlinie 3766 „Ultraschall”, welche Grenzwerte zu den zulässigen Hörschallintensitäten im Ultraschallbereich festlegen wird.
