Zusammenfassung

Zwei Fälle einer Fingernekrose nach Fingerblock-Anästhesie werden vorgestellt. Solche Zwischenfälle stellen eine sehr seltene Komplikation dieser Technik dar. Die Pathogenese und die Behandlungsmöglichkeiten werden unter Berücksichtigung der Literatur diskutiert. Durch Verwendung adrenalinfreier Anästhetika, die Infiltration von geringen Flüssigkeitsmengen auf Metakarpale-Niveau und die Anwendung einer Oberarm-Blutsperre anstelle eines Gummibandes lassen sich Durchblutungsstörungen vermeiden.

Abstract

Two cases of finger-tip necrosis following digital blocks are presented. These are rare complications of this technique. Pathogenesis and treatment options are discussed and the literature reviewed. Circulatory problems can be avoided by using adrenalinfree anaesthetics, infiltrating at the metacarpal level with small volumes, and using upper-arm tourniquets instead of rubber bands at the phalangeal level.
