Langzeitbeobachtung nach Einstellung von verlagerten Zähnen mit besonderer Berücksichtigung von Ästhetik, Parodontalstatus und Sensibilität - Ein Patientenbericht

Zusammenfassung

Die Einstellung verlagerter Zähne ist für den Kieferorthopäden immer wieder eine Herausforderung. Mit den heute zur Verfügung stehenden Techniken ist es in der Regel möglich, einen verlagerten Zahn in den Zahnbogen einzustellen. Das ästhetische Erscheinungsbild kann jedoch aufgrund einer falschen Achsenstellung, Drehständen, verlängerter klinischer Krone oder Verfärbungen beeinträchtigt sein. Dieser Patientenbericht zeigt die Behandlung und das Langzeitergebnis einer Einstellung eines aufgrund von mehreren Mesiodentes verlagerten Schneidezahnes. Die 7S-jährige Patientin stellte sich in der Poliklinik für Kieferorthopädie wegen eines palatinal durchbrechenden Zahnes im Schneidezahngebiet vor. Eine röntgenologische Untersuchung zeigte, dass dieser Zahn ein Mesiodens war. Nach der Extraktion dieses und zweier weiterer Mesiodentes begann die aktive kieferorthopädische Behandlung im Alter von 8S Jahren. Der Platz für den verlagerten Zahn wurde durch transversale Erweiterung geschaffen. Anschließend wurde der Zahn chirurgisch freigelegt und in den Zahnbogen mittels herausnehmbarer Apparatur bewegt. In der weiteren Behandlung musste der Zahn 13 ebenfalls chirurgisch freigelegt werden und eine festsitzende Apparatur für zwei Jahre eingegliedert werden. Die aktive Behandlung konnte mit 15S Jahren abgeschlossen werden. Das Ergebnis wurde noch für drei Jahre mit einem Bionator retiniert. Zehn Jahre nach der Behandlung konnte die Patientin zu einer Nachuntersuchung gewonnen werden. Das erreichte Ergebnis blieb stabil und es liegen gesunde Parodontalverhältnisse an den freigelegten Zähnen vor. Die Krone des rechten zentralen Schneidezahnes wurde etwas dunkler und röntgenologisch konnte eine Obliteration der Pulpa festgestellt werden. Die Sensibilität ist aus diesem Grunde an diesem Zahn eingeschränkt. Das Behandlungsergebnis wird bezüglich der Ästhetik, des Parodontalstatus, der Sensibilität und der Behandlungszeit diskutiert.

Summary

The treatment of impacted teeth has always been a special task for the practicing orthodontist. With the current techniques and materials it is possible to move the formerly impacted tooth to its position in the dental arch in most cases. Aesthetics might be compromised with an incorrect tooth axis, rotations, clinical elongated crown or darkening of the crown. This case report describes the treatment and long-term result of a patient with an impacted central incisor caused by several mesiodentes. The 7S year old female patient first visited the Clinic of Orthodontics of the University of Wuerzburg because of a palatinally erupting tooth. Radiological diagnostics revealed that tooth as a mesiodents and showed two other mesiodentes in that region. After the extraction of the supernumerary teeth, orthodontic treatment was initiated at the age of 8S years. After the space for the impacted central incisor was gained by transversal expansion, this tooth was surgically exposed and pulled to the occlusal plane by removable appliances. In the further course of treatment the right canine also had to be surgically exposed and a fixed appliance had to be inserted for approximately two years. Active treatment could be finished at the age of 15S years and the result was retained with a bionator for another three years.

Ten years after the end of treatment the patient could be won for another examination. The achieved result was stable and the periodontal condition at the formerly impacted teeth remained healthy, the crown of the right central incisor became a little darker due to an almost complete obliteration of the pulp. Sensibility of that tooth was restricted because of the same reason. The treatment outcome will be discussed with respect to aesthetics, periodontal status, sensibility and treatment time.
