Direkte Komposittechnik zur ästhetischen Korrektur an Frontzähnen

Adhäsive Restaurationstechniken haben in den vergangenen Jahren unverkennbare Fortschritte gemacht und mittlerweile einen hohen Stellenwert bei der Sanierung unserer Patienten erlangt. Invasive traditionelle Restaurationskonzepte werden abgelöst durch prophylaxeorientierte, non- oder minimalinvasive und gleichzeitig ästhetische Versorgungstechniken. Hohe ästhetische Ansprüche bei Frontzahnrestaurationen können durch Anwendung unterschiedlich transluzenter Kompositmaterialien befriedigt werden. Berücksichtigt man die wichtigsten Regeln der Farb- und Formgebung, so lassen sich mit der direkten Technik Ergebnisse erreichen, die guten Vollkeramikrestaurationen in nichts nachstehen. Neben der reinen Defektversorgung sind Veränderungen der Zahnfarbe, -form und -stellung in einer Behandlungssitzung chairside durchführbar. Spezielle Arbeitstechniken bei der Formgebung der Matrizen, der Schichtung und der Ausarbeitung führen zu einer sicheren Vorhersagbarkeit des Behandlungsergebnisses. An klinischen Behandlungsfällen werden Indikationsstellung und Behandlungsplanung sowie Behandlungsabläufe aufgezeigt, z. B. die Umformung von Eckzähnen in laterale Inzisivi, der Lückenschluss nach einer kieferorthopädischen Behandlung und die Formveränderung nach gingivo-parodontaler Retraktion. Die Bedeutung für die ästhetische Optimierung kieferorthopädischer Maßnahmen ist in diesem Zusammenhang besonders hervorzuheben.

Summary

Adhesive restorative techniques have distinctly improved during the last few years and have gained special importance for the treatment of our patients. Traditional invasive protocols are being replaced by non-invasive or minimally invasive and at the same time highly esthetic techniques. Anterior restorations meet even high esthetic demands when resin-based composites of different translucencies are used. Results achieved by the direct technique are equivalent to all-ceramic crowns, provided that the basic principles of shade selection and anatomic form are obeyed. In addition to mere restorations of lesions, it is also possible to modify the colour, shape and position of teeth in one visit. Special protocols regarding the shaping of matrix strips and the layering and finishing procedures of composite materials allow predictable treatment results. Diagnostics, treatment planning and therapeutic processes are demonstrated by means of clinical cases, including morphological modification of canines into lateral incisors, closure of diastemas after orthodontic treatment and modification of crown shape after the recession of gingivoperiodontal tissues. Adhesive restoration techniques therefore contribute significantly to an improvement of orthodontic treatment results.
