Zusammenfassung

Durch die Verfügbarkeit und fehlende Invasivität der Kompressionssonographie kam es in der Diagnostik der tiefen Beinvenenthrombose zu einem Wechsel der Untersuchungsindikation vom Thrombosenachweis durch die Phlebographie zum Thromboseausschluss mittels Ultraschall. Die Kompressionssonographie zeichnet sich durch eine der Phlebographie entsprechenden Sensitivität und Spezifität für die proximale Becken-/Beinvenenthrombose aus und gilt nach den aktuellen Behandlungsstudien als sichere Methode in der Diagnostik der venösen Thrombose. Durch die hohe Spezifität der Kompressionssonographie beim Nachweis einer Unterschenkelvenenthrombose und einer untersucherabhängigen Sensitivität von 50 - 95 % ist der Thromboseausschluss auch für den Unterschenkel zu fordern. Die hier dargestellten Leitlinien in der Diagnostik der Becken-/Beinvenenthrombose sind eine Empfehlung der Arbeitskreises Vaskulärer Ultraschall der DEGUM hinsichtlich Untersuchungstechnik und Dokumentation. Sie beinhalten auch einen Algorythmus zum Ausschluss einer venösen Thrombose nach den Ergebnissen aktueller Behandlungsstudien.

Abstract

As a result of availability and failing invasiveness the technique of compression sonography led to a change in the indication of thrombosis examination - from the proof of thrombosis shown by phlebography to exclusion of thrombosis by ultrasound. Compression sonography showed the same sensitivity and specificity as phlebography for proximal limb thrombosis and reaches high safety due to actual treatment studies in the diagnosis of deep vein thrombosis. For the distal limb the compression sonography has provided > 90 % specificity, with an examinator dependent sensitivity of 50 to 95 %, and hence examination of the distal limb must be recommended. These guidelines for the diagnosis of deep vein thrombosis are recommendations by the Section of Vascular Ultrasound of DEGUM for examination technique and documentation. It also contains an algorithm of diagnosis for thrombosis as a result of the actual treatment studies.
