Zusammenfassung:

Der Artikel beschreibt eine mögliche Vorgehensweise im Rahmen einer kombiniert kieferorthopädisch-kieferchirurgischen Behandlung von Patienten, die dem “short-face-syndrome” (Klasse II, skelettal tiefer Biss und kurzes Untergesicht) zuzuordnen sind. Dieses Verfahren, bei dem bewusst auf das präoperative Nivellieren des Unterkieferzahnbogens verzichtet wird, ermöglicht ein ansprechenderes ästhetisches Behandlungsergebnis, da neben der Vorverlagerung des Unterkiefers auch eine Verlängerung des Untergesichts erzielt wird. Da durch die Rotation des anterioren Segments die Muskulatur im Rahmen der Bisshebung in geringerem Ausmaß gestreckt wird als bei einem Verfahren mit reiner Ventraltranslation des anterioren Unterkiefersegments, ist diese Vorgehensweise auch weniger anfällig für Rezidive.

Summary

The following article demonstrates an alternative combined orthodontic-surgical treatment concept in patients with short-face-syndrome (class II deformities, skeletally deep bite and short lower face).

By avoiding the preoperative orthodontic levelling of the lower arch the necessary advancement of the mandible and lengthening of the lower face is realised simultaneously by rotating the anterior segment instead of just translating the mandible straight forward. This does not only result in a positive effect on extraoral demands but also decreases the tendency of relapses.
