Seit den grundlegenden Arbeiten von Furchgott und Zawadzki im Jahr 1980 ist die herausragende Rolle des Endothels bei der Acetylcholin-induzierten Vasodilatation etabliert [20]. Der propagierte „diffusible” Faktor, welcher die endothelabhängige Vasodilatation hervorruft, wurde später als Stickstoffmonoxid (NO) identifiziert. NO wird aus L-Arginin synthetisiert, eine Reaktion, die durch Isoenzyme der NO-Synthase (NOS) katalysiert wird [34]. Die endotheliale NO-Synthase (eNOS) ist in Zusammenhang mit der endothelabhängigen Vasodilatation von besonderem Interesse. Den Begriff „Endothelfunktion” aber lediglich mit der endothelabhängigen Vasodilatation gleichzusetzen wäre angesichts der zahlreichen weiteren Aufgaben des Endothels, unter anderen dem Beitrag zur Thrombogenese, zur Thrombolyse, zur Antiinflammation und Antiproliferation, zu kurz gefasst. Dennoch soll sich diese Übersicht auf die endothelabhängige Vasodilatation beschränken, da hierzu in-vivo-Daten beim Menschen existieren.

Beim Menschen kann die endothelabhängige Vasodilatation in Anlehnung an die klassischen Experimente von Furchgott und Zawadzki durch lokale Gaben von Acetylcholin und einer Erfassung der Vasodilatation in nachgeschalteten Stromgebieten mit entsprechender Zunahme der Durchblutung untersucht werden. Dieses Verfahren ist unter anderem für den Koronarkreislauf und das Unterarmstromgebiet etabliert (Abb. [1]). Eine gestörte endothelabhängige Vasodilatation gilt als frühe Manifestation atherosklerotischer Erkrankungen verschiedener Genese und konnte in den genannten Stromgebieten zum Beispiel bei Hypercholesterinämie [27] [50] und arterieller Hypertonie [17] [35] eindrucksvoll demonstriert werden.

eNOS wird auch in der Niere exprimiert. Ihr Nachweis gelang unter anderem in den Glomeruli, den Arteriolen und in der Macula densa [2]. Im Nierenstromgebiet gibt es fundierte Hinweise dafür, dass aufgrund der vielfältigen Wirkungen von NO eine gestörte endothelabhängige Vasodilatation zur Genese und/oder zur Progression verschiedener Nierenerkrankungen beiträgt [37].

Unabhängig von der Funktion des Endothels kann eine Relaxation der glatten Muskelzellen auch durch die Gabe von NO-Donatoren wie Nitroprussid erreicht werden.

Die Untersuchung der endothelabhängigen Vasodilatation im Nierenstromgebiet des Menschen gestaltet sich ungleich schwieriger als in den anderen Körperbereichen, da Acetylcholin nur lokal nach der selektiven Katheterisierung einer Nierenarterie gegeben werden kann. Im Gegensatz zum Unterarmstromgebiet ist dieses Verfahren wesentlich invasiver. Während im koronaren Stromgebiet selektive Katheterisierungen einzelner Koronarien Routine sind und somit bei einem Patienten sowohl gesunde als auch atherosklerotisch erkrankte Gefäße untersucht werden können, ist dies bei der Nierenarterie so nicht ohne weiteres möglich. Eine systemische Gabe von Acetylcholin zur Analyse der Endothelfunktion im Nierenstromgebiet ist wegen der parallelen Auswirkungen auf die systemische Hämodynamik nicht hilfreich. Vor diesem Hintergrund ist es nicht verwunderlich, dass bislang nur in sehr wenigen Arbeiten eine direkte radiologische Erfassung des Effektes von Acetylcholin auf die Vasodilatation der Nierenarterie des Menschen untersucht wurde [19] [48] . Dopplersonographische Verfahren sind derzeit keine validen Alternativen zu den radiologischen Verfahren, da die Nierenarterie sonographisch in ihrem Querschnitt letztlich nicht visualisiert werden kann und sich Aussagen zur Perfusion somit auf Flussgeschwindigkeiten und Widerstandsindizes beschränken müssen [28].

kurzgefasst: Stickstoffmonoxid (NO) wird im Nierenstromgebiet vom Endothel synthetisiert und trägt zur Regulation der Nierenperfusion bei. Die funktionelle Untersuchung am Menschen gestaltet sich aber schwierig.
