Zusammenfassung

Die Operation nach Kapandji-Sauvé beinhaltet die Arthrodese des distalen Radioulnargelenks (DRUG) in Kombination mit einer Segmentresektion der distalen Ulna. Das Verfahren wird neben alternativen Operationen bei der Funktionsstörung des DRUG angewendet. Ziel dieser retrospektiven Studie waren die Analyse der Operationsindikation, die Erfassung der subjektiven, funktionellen und radiologischen Ergebnisse nach erfolgter Kapandji-Sauvé-Operation sowie die Erfassung der Früh- und Spätkomplikationen.

Die Ergebnisse basieren auf 56 Operationen, die zwischen 1989 und 2002 erfolgten. Der mittlere Nachuntersuchungszeitraum betrug 5,9 ± 3,6 Jahre. 15 von den 56 Operationen erfolgten vor 1996. Indikation für die Operation war am häufigsten die Arthrose oder Dislokation im DRUG nach Radiusfraktur. Bei 94 % der Patienten konnte eine Schmerzreduktion erzielt werden. Eine uneingeschränkte Belastbarkeit des Armes wurde jedoch nur bei 53 % erreicht. Vier Patienten zeigten klinische Zeichen eines radio-ulnaren Impingement-Syndroms. Ein kurzer proximaler Stumpf korrelierte mit einem schlechteren Mayo-Wrist-Score-Ergebnis (p = 0,018) und einem höheren Schmerzniveau (p = 0,046). Die Zunahme des Bewegungsumfangs des Handgelenks war in allen Ebenen signifikant. Der postoperative DASH-Wert betrug 22,6 ± 20,0 Punkte, der Mayo-Wrist-Score 79,5 ± 14,6. Als Frühkomplikationen waren die Ausbildung einer Pseudarthrose des DRUG mit Schraubenbruch und die Ausbildung einer Reflexdystrophie zu beobachten. Als einzige Spätkomplikation zehn Jahre postoperativ war eine beginnende Arthrose im Humero-radial-Gelenk festzustellen.

Mit der Kapandji-Sauvé-Operation besteht bei entsprechender Indikation auch langfristig eine zuverlässige und komplikationsarme Therapie für die Behandlung der bewegungseinschränkenden Arthrose oder Dislokation des DRUG.

Abstract

We present the preliminary results of a retrospective study on 56 patients who underwent the Kapandji-Sauvé procedure for chronic disorders of the distal radioulnar joint (DRUJ). Outcome was assessed with special regard to the long-term results.

The average follow-up was 5.9 years (1 to 12 years). 15 of the 56 operations were performed before 1996. Most procedures were performed because of secondary arthrosis or chronic dislocation of the DRUJ after distal radius fracture.

Patients were assessed for pain, range of motion of wrist and forearm and radiological features. The DASH score and Mayo wrist score were used.

Pain was improved in 94 % of the patients, but only 53 % were free of symptoms during heavy manual labour concerning the operated site. In four cases symptoms of ulnar impingement were found. Improvement in range of motion of wrist and forearm was significant. The post-operative DASH score was 22.6 ± 20.0 and the Mayo wrist score was 79.5 ± 14.6. One non-union of the DRUJ with consecutive fracture of the fixation screw and an algodystrophy in another case were found as postoperative complications. The only long-term complication consisted of a beginning humeroradial arthrosis ten years after the operation.

The results demonstrate high patient satisfaction and reliable improvement in range of motion. Our results confirm the Kapandji-Sauvé procedure to be a reliable salvage procedure for arthrosis or chronic dislocation of the DRUJ even after long-term follow up.
