Zusammenfassung
Vorgestellt wird eine Patientin mit einem Merkel-Zell-Karzinom an der hinteren Kommissur
der Vulva, die durch eine weite Exzision, beidseitige inguinale Lymphonodektomie und
postoperative lokoregionäre Strahlentherapie behandelt wurde. Ein Jahr nach Erstdiagnose
verstarb die Patientin an den Folgen einer diffusen Leber- und Knochenmetastasierung.
Aufgrund des extrem seltenen Tumorsitzes an dieser Stelle, es sind in der Literatur
gerade 12 Fälle dazu publiziert, sollten die Therapieempfehlungen zum extragenitalen
Merkel-Zell-Karzinom auf die gynäkologische Manifestation übertragen werden. Danach
sollte bei lokal oder regionär begrenzter Tumorausdehnung (Stadium I und II) eine
großzügige Exzision mit inguinaler Lymphknotenexstirpation erfolgen und eine postoperative
Strahlentherapie hinzugefügt werden. Eine Chemotherapie ist bei Fernmetastasen (Stadium
III) angezeigt.
Abstract
A case of Merkel cell carcinoma of the vulva is presented. The tumor was 1 cm in diameter
and located in the left posterior fourchette. After wide excision of the tumor and
bilateral inguinal lymphadenectomy the patient underwent irradiation therapy. After
9 months several liver metastases were noted and she died one year after primary diagnosis.
In this location such a tumor is extremely rare and only 12 case reports have been
published to date. Therefore general recommendations for therapy of Merkel cell carcinoma
should be applied to this location. Accordingly, in stage I and II wide excision or
vulvectomy with lymphadenectomy should be performed. To reduce the risk of recurrence
irradiation should be added. In stage III chemotherapy is indicated.
