Cytoreduktive surgery before high dose Interleukin-2 based therapy in patients with metastatic renal cell carcinoma

Abstract

Ausgewählte Patienten mit einem metastasierten Nierenzellkarzinom wurden vor geplanter systemischer Hochdosis-Interleukin-2-Therapie radikal nephrektomiert. In einem 11-jährigen Beobachtungsintervall wurden 195 Patienten operiert. Die perioperative Mortalität lag bei 1 %, die perioperative Komplikationsrate bei 12,8 %. Die Interleukintherapie wurde bei 107 Patienten (55 %) durchgeführt. Gründe für den Ausschluss von einer Immuntherapie waren Tumorprogress (n = 44; 22,6 %), perioperative Komplikationen (n = 18, 9,2 %), fehlender Tumorprogress (n = 8; 4 %) und andere Tumorentitäten in der Histologie (n = 5;2,6 %). Die Ansprechrate lag in der Gruppe mit Immuntherapie bei 17,8 % (n = 19). Zytoreduktive Chirurgie vor geplanter Immuntherapie ist mit einer tolerablen Komplikationsrate verbunden und kann zu einem anhaltenden Tumorregress führen.
