Zusammenfassung

Es wird über einen 32-jährigen Patienten mit einer unteren gastrointestinalen Blutung als Erstmanifestation eines M. Behçet berichtet. Es handelt sich bei diesem Krankheitsbild um eine chronisch-entzündliche Systemerkrankung, deren Diagnose im Rahmen der Erstmanifestation schwierig ist, insbesondere im Hinblick auf die differenzialdiagnostische Abgrenzung zu chronisch-entzündlichen Darmerkrankungen. Zudem ist die untere gastrointestinale Blutung eine seltene Erstmanifestation dieser Erkrankung. Es wurde eine systemische, immunsuppressive Therapie begonnen und erst der Krankheitsverlauf mit rekurrierenden oralen Ulzerationen gab eindeutige Hinweise auf die Diagnose. Weitere Blutungsepisoden oder gastrointestinale Symptome konnten während einer Beobachtungszeit von 36 Monaten nicht mehr beobachtet werden. Im Rahmen der Diskussion werden diagnostische Probleme und therapeutische Ansätze des Krankheitsbildes bei einer Abgrenzung von benachbarten Entitäten aufgezeigt.

Abstract

A case of a 32-year-old male patient with lower gastrointestinal bleeding is reported as an initial manifestation of Behçet’s disease. The early diagnosis of this chronic-systemic disease is often very difficult, in particular with regard to the differentiation from chronic inflammatory bowel diseases. In addition, lower gastrointestinal bleeding is a rare manifestation of Behçet’s disease. An immunosuppressive therapy was initiated and the course of the disease with recurrent oral ulcerations gave evidence for the diagnosis. Further bleeding episodes or other gastrointestinal symptoms have not been observed during a follow-up period of 36 months. The diagnostic problems and the therapeutic management of the disease are discussed in relation to neighbouring entities.
