Zusammenfassung

In dieser Übersichtsarbeit sind wesentliche und klinisch relevante neuere Arbeiten auf dem Gebiet der postoperativen Schmerztherapie dargestellt und bewertet. Aus den Studien lassen sich verschiedene Konsequenzen für die klinische Arbeit ziehen: i) die postoperative Gabe von NSAID führt zu signifikant mehr operationspflichtigen Nachblutungen nach Tonsillektomien und sollte bei diesem Eingriff daher zurückhaltend erfolgen; ii) die zusätzliche Gabe von COX-2-Inhibitoren zu einer postoperativen i. v. Analgesie mit Opioiden verbessert das funktionelle Outcome nach Kniegelenksendoprothetik; iii) die Kombinationstherapie verschiedener Nicht-Opioidanalgetika ist nicht hinreichend belegt und sollte daher in der Routine weiterhin nicht angewandt werden; iv) bei der postoperativen Opioidtitration ist der Bedarf vom Lebensalter unabhängig; in der anschließenden Dauertherapie ist der Bedarf dann aber signifikant geringer; v) Morphin und Piritramid sind gleich gut analgetisch wirksam und erzeugen in gleichem Maße Übelkeit und Erbrechen, so dass die Auswahl des Opioids für die postoperative Schmerztherapie nach Kostengesichtspunkten erfolgen kann, und vi) falls die postoperative Analgesiequalität von Tramadol nicht ausreichend ist, kann ursächlich eine Enzymvariante im Cytochrom-Oxidase P450 Gen (CYP2D6) verantwortlich sein; die betroffenen Patienten sollten auf ein anderes Opioid umgestellt werden.

Abstract

In this review, novel clinical studies on postoperative pain therapy are summarized. Based on these studies, several conclusions can be drawn: i) following tonsillectomy, postoperative therapy with NSAIDs leads to a significant increase in the number of reoperations; thus NSAIDs should be used with caution; ii) COX-2 inhibitors in combination with intravenous opioids improve recovery and functional outcome after knee replacement surgery; iii) the combination therapy of different non-opioid analgesics has no proven clinical efficacy and should not be used routinely; iv) patients' age is not a determinant in postoperative opioid titration after surgery; in contrast, it does predict opioid consumption during the first postoperative day; v) morphine and piritramide have identical analgesic efficacy and induce nausea and vomiting with the incidence; opioid selection can, thus, be based on economic considerations and vi) if tramadol is ineffective in postoperative pain therapy, this might be caused by an allelic variant of one of the cytochrome P450 enzymes (CYP2D6); these patients should be treated with a different opioid.
