Zusammenfassung

Fragestellung: Ist die Morphologie und Funktion des weiblichen Beckenbodens nach primärer und sekundärer Sectio caesarea im Vergleich zu Nulliparae verändert?

Patientinnen und Methodik: Im Rahmen einer retrospektiven Analyse wurden 20 Nulliparae, 21 Patientinnen nach primärer Sectio (Gruppe PS) und 22 Patientinnen nach sekundärer Sectio caesarea (Gruppe SS) im Mittel 424 bzw. 517 Tage postoperativ klinisch, urodynamisch und durch funktionelle Magnetresonanztomographie (MRT) untersucht.

Ergebnisse: Die Patientinnen aus der Gruppe PS gaben im Mittel 0,7 Inkontinenzereignisse/Woche (Gruppe SS 2,4) und einen häufigeren imperativen Harndrang an (Gruppe PS: 29 %, Gruppe SS: 27 %). Der Unterschied war jeweils signifikant gegenüber den Nulliparae. Klinisch war jeweils nur der ICS-Punkt Bp signifikant tiefer deszendiert. In der MRT zeigten die Patientinnen von Gruppe PS und SS einen engeren knöchernen Geburtskanal, eine tiefere Position des Blasenhalses beim Pressen, eine größere Rektozele und eine größere Fläche des posterioren Levatortors.

Schlussfolgerungen: Nach primärem und sekundärem Kaiserschnitt sind mithilfe von Anamnese, klinischer Untersuchung und funktioneller MRT morphologische und funktionelle Veränderungen am Beckenboden im Vergleich zur Kontrollgruppe nachweisbar.

Abstract

Purpose: In this study the morphological and functional changes of the female pelvic floor after primary and secondary caesarean section were examined. The two groups were compared with each other and a control group of nulliparous women.

Patients and Methods: 20 nulliparous women, 21 women after primary and 22 after secondary caesarean section were examined, including medical history, gynaecological examination, urodynamics and functional MRI of the pelvic floor. Mean postoperative follow-up was 424 and 517 days, respectively.

Results: Compared with nulliparous women, a significantly higher percentage of patients reported a history of urinary incontinence and urgency. After primary and secondary caesarean section the clinical ICS Score showed lower values for point BP. By means of functional MRI, a smaller obstetric canal, a higher mobility of the vesical neck during valsalva maneuver and a more prominent rectocele was demonstrated in these patients. Furthermore we could demonstrate an increase of the posterior area of the levator hiatus.

Conclusions: Pregnancy itself leads to morphological and functional changes of the pelvic floor.
