Definition, Epidemiologie

Rund 6 Millionen (7,5 %) Deutsche leiden an einem Diabetes mellitus, weltweit etwa 150 Millionen. Die Prävalenz vor allem des Typ 2 Diabetes steigt stetig an. So fand sich im Zeitraum von 1999 - 2001 in Deutschland ein kontinuierlicher Anstieg der Zahl von Patienten mit bekanntem Diabetes mellitus um etwa 5 % pro Jahr [22]. Bis zum Jahr 2025 wird weltweit mit einer Verdoppelung der Prävalenzrate gerechnet [20] [40] [49] [53]. Häufig ist der Diabetes mellitus Bestandteil des metabolischen Syndroms, einem Konglomerat kardiovaskulärer Risikofaktoren, das neben dem Diabetes mellitus Übergewicht und Adipositas, Hypertonie, Dyslipoproteinämie und Hypertriglyzeridämie umfasst. Dieses metabolische Syndrom kann heute bereits bei 20 - 30 % der erwachsenen Mitteleuropäer festgestellt werden und wird zunehmend auch bei Kindern angetroffen [5] [58]. Nach Daten aus dem Jahr 1998 betrug in Deutschland der Kostenaufwand für einen Typ-2-Diabetiker jährlich umgerechnet ca. 4500 EUR, was Gesamtkosten von ca. 16 Milliarden EUR ergibt. Die Kosten stiegen dabei auf das 4,1-Fache, wenn makro- und mikrovaskuläre Komplikationen vorlagen [35].

In Deutschland treten jährlich ca. 200 000 Schlaganfall-Neuerkrankungen auf. Nahezu jeder fünfte Betroffene stirbt innerhalb der ersten 4 Wochen. Ca. 1 Million Menschen sind durch Schlaganfall betroffen und haben zum Teil ausgeprägte residuale Beeinträchtigungen. Der Schlaganfall ist somit die häufigste Ursache für bleibende Behinderungen [24] [32]. Die Kosten der Akutbehandlung eines Schlaganfallpatienten werden derzeit auf 5000 - 8000 EUR geschätzt [18] [57]. Beide Erkrankungen, Diabetes und Schlaganfall, stehen miteinander in Wechselbeziehung, die im Folgenden näher dargestellt werden soll.
