Einleitung

Die Aufgabe der Notfall- und Intensivmedizin besteht in der Diagnose, Überwachung und Therapie kritischer Abweichungen der Homöostase. Der Schock stellt die Maximalvariante einer Dysregulation der fein aufeinander abgestimmten Regelsysteme des Organismus als Reaktion auf ein initial schädigendes Ereignis dar. Im Zentrum steht hierbei der kritische Abfall des Herzzeitvolumens (HZV) bzw. das inadäquate HZV mit konsekutiver Mikrozirkulationsstörung und inadäquater Versorgung der einzelnen Organsysteme und Gewebe [1]. Allen Schockformen gemeinsam ist ein Missverhältnis von Sauerstoffangebot und Sauerstoffverbrauch [2].

Die adäquate Behandlung des Schocks ist einer, der Hauptaufgaben des Notfall- und Intensivmediziners. Nur durch eine möglichst frühe, schnelle und zielgerichtete Therapie kann der Schockzustand erfolgreich behandelt werden. Unbehandelt verläuft dieses Krankheitsbild in der Regel tödlich.

Auch wenn die gemeinsame Endstrecke des Schocks die Hypotonie mit Hypoperfusion der Organe ist, bestehen in der auslösenden Ursache doch erhebliche Unterschiede. Die differenzierte Kenntnis der Pathogenese und Pathophysiologie der einzelnen Schockformen hat eine erhebliche Bedeutung für die Diagnostik und vor allem für eine erfolgreiche Therapie [2] [3].

Die nachfolgende Übersicht vermittelt einen Überblick über aktuelle Therapiemöglichkeiten einer myokardial bedingten Unterversorgung der Organe und Gewebe mit Sauerstoff.
