Medikamentöse Behandlung des operierten Klappenpatienten

Einleitung

Bei derzeit ca. 18 000 Implantationen pro Jahr und bekannten postoperativen Mortalitätsraten erscheint eine Zahl von ca. 170 000 Kunstklappenträgern realistisch. Die medikamentöse Therapie dieser Patienten ist abhängig von:

der Art des chirurgischen Eingriffs (klappenerhaltend oder Klappenersatz), ggf. der Prothesenwahl und der kardialen Morphologie und Physiologie (z. B. Myokardfunktion, Rhythmus).

Eine postoperativ persistierende myokardiale Dysfunktion erfordert eine konsequente Herzinsuffizienzmedikation. Da mechanische Kunstklappen thrombogen sind, ist eine dauerhafte orale Antikoagulation unerlässlich. Mechanische und biologische Herzklappenprothesen prädisponieren für polymerassoziierte Infektionen insbesondere durch koagulasenegative Staphylokokken, sodass die Leitlinien zur Endokarditisprophylaxe zu befolgen sind.
