Zusammenfassung

Die bisherige medikamentöse Behandlung der Endometriose zielt im Wesentlichen auf eine ovarielle Supression mittels Östrogen-Gestagen-Kombinationen (Kontrazeptiva), bevorzugt im Langzyklus, Gestagen-Mono-Therapie (auch als Gestagenspirale, Mirena®), GnRH-Agonisten und Danazol. Daneben kommen noch verschiedene Schmerzmittel (NSAID) zum Einsatz. Nach dem Absetzen der medikamentösen Therapie ist die Rezidivrate jedoch sehr hoch. Leider gibt es bisher keine Medikamente, die eine Heilung der chronischen Erkrankung Endometriose bewirken könnten. Für viele Patientinnen kann mit der bisher bekannten Medikation eine ausreichende Therapie der Endometriose erreicht werden, und so werden Medikamente aus diesen Substanzgruppen sicher auch in der Zukunft weiterhin in großem Umfang zum Einsatz kommen. Bei einigen Patientinnen kommt es jedoch trotz dieser Therapie zum Endometrioserezidiv und/oder keiner ausreichenden Schmerzkontrolle. Andere Patientinnen leiden unter den verschiedensten Nebenwirkungen dieser bekannten Medikamente und fragen nach neuen Therapieansätzen. Ziel unseres Beitrages ist es deshalb, neue Therapiekonzepte vorzustellen. Diese umfassen Angiogeneseinhibitoren, Aromatasehemmer, GnRH-Antagonisten, Cyclooxygenase-2-Hemmer, ER2-Agonisten, selektive Progesteronrezeptormodulatoren (SPRMs), selektive Estrogenrezeptormodulatoren (SERMs) und Thiazolidinedione. Wir berichten unter anderem von den neuesten Vorträgen des Weltendometriosekongresses in Maastricht im September 2005. Hierbei wurden zum Teil tierexperimentelle Studien vorgestellt, zum anderen Teil klinische Studien der Phase II und III. Wichtig für die Praxis ist, dass alle diese neuen Therapieformen bisher nicht zur Behandlung der Endometriose zugelassen sind, sodass eine ausführliche Aufklärung über den Off-Label-Use erfolgen muss. Damit verbunden ist, dass dadurch auch die Kosten der Therapie häufig von den Krankenkassen nicht übernommen werden. Weitere Studien sind erforderlich, aber einige viel versprechende Ergebnisse lassen auf die Zukunft hoffen.

Abstract

The medical treatment of endometriosis to date has mainly focused on suppression of the ovarian function with a combination of estrogen and gestagen (oral contraceptives), preferentially in a long cycle, gestagen only therapy (also as gestagen-IUD, Mirena®), GnRH-agonists and danazol. Additionally, different pain killers (NSAID) are used. After cessation of therapy, the recurrence rate is very high. Unfortunately, no drugs exist which could provide a definite cure of the chronic disease endometriosis. For many patients, the present medication is sufficient for the treatment of endometriosis, and so the drugs from these substance groups will be widely used in future. But for some patients, even with this medical treatment, recurrence of endometriosis will appear, or pain relief will be insufficient. Other patients suffer from different side effects of these known drugs, and they ask for new treatment options. The goal of our report is therefore to present new therapy modalities. These include angiogenesis inhibitors, GnRH-antagonists, cyclooxygenase2-inhibitors, ER2-agonists, selective progesterone receptor modulators (SPRMs), selective estrogen receptor modulators (SERMs) and thiazolidinediones. We also report on presentations from the world endometriosis meeting in Maastricht in September 2005. Animal studies and clinical phase II and III studies were presented there. Important for clinical practice is that all these new treatment modalities are not approved for the treatment of endometriosis, so that we recommend a detailed education about the off-label use of these drugs. In addition, the cost of these treatments is usually not covered by health insurance. Future studies are necessary, but promising results let us hope for the future.
