Internationale Standards der Kosten-Nutzen-Bewertung

Zusammenfassung

Dem internationalen Trend folgend beabsichtigt Deutschland die Kosten-Nutzen-Bewertung gesetzlich zu verankern, um über Erstattungsfähigkeit und Preise von Arzneimitteln zu entscheiden. Diese Übersichtsarbeit stellt Methoden der pharmakoökonomischen Evaluation dar, die international bei der Post-Marktzulassungsevaluation angewandt und diskutiert werden. Die Anforderungen der Länder an gesundheitsökonomische Analysen decken sich in vielen Bereichen: Als geeigneter Komparator gilt die kosteneffektivste Therapie. Als pragmatischen Ansatz akzeptieren die Institutionen jedoch regelhaft die derzeit übliche bzw. günstigste Behandlungspraxis. Die Kosten-Nutzwert-Analyse mit Messung der gesundheitlichen Ergebnisse durch qualitätsadjustierte Lebensjahre ist die am häufigsten empfohlene pharmakoökonomische Analyseform. Auch besteht Konsens, dass pharmakoökonomische Studien, wenn nicht primär, so zumindest zusätzlich aus gesamtgesellschaftlicher Perspektive vorzunehmen sind. Um auch bei quantitativem oder qualitativem Datenmangel Empfehlungen aussprechen zu können, verwenden die Institutionen umfassende gesundheitsökonomische Modellierungstechniken. Bei methodischen Einzelaspekten, z. B. der Kostenkalkulation, jedoch steht ein international konsentierter Standard noch aus.

Abstract

Following an international trend, Germany intends to establish cost-effectiveness evaluation in order to decide on reimbursement and pricing of pharmaceuticals. This review describes methods of comparative pharmacoeconomic analysis that are used and discussed internationally in the context of post-licensing evaluation. Most of the countries’ requirements for pharmacoeconomic analyses are similar: The most cost effective therapy is regarded as appropriate comparator. Pragmatically however, institutions accept that the drug is compared to the treatment of daily routine or to the least expensive therapy. Cost utility analysis measuring health outcomes in terms of Quality Adjusted Life Years (QALY) is most often recommended as appropriate analytic design. There is also consensus that pharmacoeconomic studies should be, at least additionally, carried out from a societal perspective. In the case of lacking data - in quantitative as well as in qualitative terms - review bodies use economic modelling techniques to support reimbursement and pricing decisions. However, specific methodological issues, e. g. concerning cost calculation, still lack internationally consented standards.
