Zusammenfassung
Orthodontische Zahnbewegungen haben das Ziel, Fehlstellungen im Wechsel- und Erwachsenengebiss
zu korrigieren und die Prognose parodontal erkrankter Zähne zu verbessern. Neue regenerative
parodontale Behandlungsmöglichkeiten (Membrantechnik, Applikation von Schmelz-Matrixproteinen)
bieten günstigere Voraussetzungen, neues Attachment zu gewinnen und haben die präorthodontischen
Bedingungen für Zahnbewegungen in infraalveoläre Defekte oder die vertikale Bewegung
von Zähnen mit reduziertem Knochenangebot verbessert. Nutzen und Probleme eines kombinierten
parodontal/orthodontischen Behandlungsansatzes werden in Hinblick auf präorthodontischen
Weichgewebsaufbau, gesteuerte Geweberegeneration, Verlust interdentaler Gingiva, Korrektur
von Engständen, Reorientierung gewanderter aufgefächerter Frontzähne und Gewinn neuer
Pfeilerzähne durch Distalisation von endständigen Prämolaren diskutiert.
Abstract
Orthodontic tooth movement aims at correcting malpositioned teeth in the mixed and
adult dentition and improving the prognosis of periodontally affected teeth. New regenerative
periodontal treatment procedures (membrane technique, application of enamel matrix
derivates) have provided greater opportunities for gaining new attachment and have
improved the pre-orthodontic conditions for moving teeth into infrabony defects or
for vertical movements of teeth with reduced bone support. The benefits and problems
of a combined periodontal-orthodontic treatment approach are discussed with respect
to preorthodontic mucosal grafting, guided tissue regeneration, loss of interdental
gingiva, correction of crowding, reorientation of migrated and flared incisors, and
gaining new abutment teeth by distalizing free-end premolars.
