Summary

There is evidence from a small number of randomized controlled trials (class of evidence I/II) to suggest that the use of bone morphogenic proteins (rhBMP-2 or rhBMP-7) in acute tibial fractures may:

Increase union and wound healing, reduce the need for secondary procedures due to nonunion or delayed union, lower rates of hardware failure and result in less pain compared with IM nailing alone.

Result in similar outcomes compared with autologous bone grafting.

The use of rhBMP-7 for treatment of persistent tibial nonunion compared with autograft resulted in lower rates of device complication and osteomyelitis and less operative blood loss. Differences between groups for nonunion, pain, infection and weight bearing were similar between groups receiving BMP or autologous bone graft.
