Zusammenfassung
Die fiberoptische Intubation ist eine anerkannte Methode zur Durchführung der „schwierigen Intubation”. In der Mehrzahl der Fälle muß aufgrund der vorliegenden anatomischen Bedingungen der transnasale Zugang gewählt werden. Wir berichten über einen Fall von fiberoptischer Intubation bei behinderter Nasenpassage. Unter Verwendung eines Oraltubus nach Williams und einer Patil-Syracuse-Maske wurde der Patient erfolgreich fiberoptisch orotracheal intubiert. Die vorgestellte Technik empfiehlt sich für solche Fälle, in denen die fiberoptische Intubation indiziert, die transnasale Passage aber nicht sinnvoll oder nicht möglich ist und die Intubation in Narkose durchgeführt werden soll.
Summary
Fiberoptic intubation is widely accepted in the management of a "difficult airway". In the majority of these cases the underlying anatomical findings require a nasal approach. We report a case of fiberoptic intubation where nasal passage was not possible. Orotracheal fiberoptic intubation was easily performed using a Williams oral airway and a Patil-Syracuse face mask. We recommend this technique for those cases where fiberoptic intubation is indicated but contraindications to the transnasal route apply and an „asleep-intubation” is desired.
