Dopplersonographische Beurteilung der Blutströmungsrichtung bei distalem Verschluß der Unterschenkelarterien*

Zusammenfassung

Bei der Messung des Unterschenkelarterien-druckes mit Hilfe der Cw-Doppler-Methode ist es erforderlich, die Strömungsrichtung am Signalableitungspunkt zu beachten. A.-tibialis-anterior-Verschlüsse können über den Arcus planta-ris aus der A. tibialis posterior kollateral versorgt werden, so daß es zur Stromrichtungsumkehr in der A. dorsalis pedis kommt. Bei Nichtbeachtung diescr Möglichkeit wird es zur Fehleinschätzung des A.-tibialis-anterior-Druckes kommen. Ein Komprcssionstest der A. tibialis posterior kann die Durch-blutungssituation klären. Das gilt sinngcmäß auch für die Ver-schlüsse der übrigen Unterschenkelarterien.

Abstract

It is necessary to observe the direction of the blood flow for measurement of the ankle pressure by the Dopplermethod. Obliteration of the anterior tibial artery can be compensated by collaterals from the posterior tibial artery. Through this the blood flow is reversed in the dorsalis pedis artery. If this possibility is disregarded, the pressure of the anterior tibial artery will yield false data. The situation of the blood flow can be clarified by a compression test of the posterior tibial artery. This also applies to obliterations of the other arteries of the ankle.
