Zusammenfassung

  1. Die Reproduzierbarkeit völlig atraumatischer Flußmessungen der A. femoralis mit Hilfe eines 13kanaligen gepulsten Doppler-Ultraschall-Blutflußmeßgerätes wurde durch Wiederholungsmessungen am selben Tag und an 3 verschiedenen Tagen bei Gesunden und Patienten mit peripheren Arterienverschlüssen überprüft. - Der Variationskoeffizient bei Wiederholungsmessungen des elektronisch gemittelten Flusses, des systolischen Flußmaximums bzw. der Flußamplitude betrug am selben Tag 13.6, 7.1 bzw. 6.4 und an 3 verschiedenen Tagen 23.9, 17.0 bzw. 16.4. Die Reproduzierbarkeit wird durch physiologische Schwankungen einerseits und die Schwierigkeit der Einstellung des Gefäßes (Geometrie des Meßgebietes) bestimmt.

  2. Normalwerte der folgenden Flußparameter der A. femoralis wurden an einem gesunden Kollektiv (N = 20, Alter = 39 ± 18 Jahre) ermittelt (ml/sec): Durchschnittsfluß 4.4 ± 0.6, systolisches Flußmaximum: 24.0 ± 4.5, systolisches Flußmaximum: - 7.0 ± 3.6, diastolisches Flußmaximum: 3.6 ± 1.6.

  3. Bei Patienten mit angiographisch definierter Stenose der A. iliaca (N = 12, Alter: 62 ± 7.5 Jahre) war das systolische Flußmaximum mit 10.3 ± 3.1 vermindert (p < 0.001) und der spätsystolische Rückfluß aufgehoben - systolisches Flußminimum nunmehr + 3.9 ± 2.1 - während der Durchschnittsfluß und das diastolische Flußmaximum nicht signifikant verändert waren (p < 0.05).

  4. Die Methode ist aufgrund obiger Befunde geeignet zur diagnostischen Beurteilung des Flusses der A. femoralis - z.B. Beurteilung der Dignität vorgelagerter Stenosen - und zur akuten und Langzeitbeurteilung therapeutischer Maßnahmen wie Gefäßchirurgie, Thrombolyse, Katheterrekanalisation, vasoaktive Pharmaka bzw. Änderung der Blutviskosität.

Abstract

  1. The reproducibility of atraumatic quantitative measurements of the blood flow of the femoral artery has been estimated using a 13-cannel pulsed Doppler ultrasound flow meter. Repetitions of the measurements were done in the same subjects 5 times at the same day and in 3 different days of the same week respectively. - The coefficient of variance of the mean flow, the systolic peak flow and the flow amplitude was 13.6, 7.1 and 6.4 in measurements of the same day and 23.9, 17.0 and 16.4 in measurements of different days. The reproducability was limited by the physiological variance and the difficulty of adapting the flow probe to the vessel in the same way (geometry of measurement).

  2. Normal values of the following parameters of the flow of the femoral artery were measured in a group of 20 healthy subjects (age 39 ± 18 years): mean flow: 4.4 ± 0.6, systolic peak flow: 24.0 ± 4.5, systolic minimal flow: - 7.0 ± 3.6, diastolic maximal flow: 3.6 ± 1.6 ml/sec each.

  3. In a group of 12 patients with Stenosis of the homolateral iliac artery (angiography) (mean age 62 ± 7.5 years) the systolic peak flow was decreased (10.3 ±3.1) and the reflux in the late Systole was abolished (systolic minimal flow: + 3.9 ± 2.1) (p < 0.001), whereas the mean flow (4.6 ±1.9) and the maximal diastolic flow (4.2 ± 1.8) were not changed (p < 0.05).

  4. The method might be useful for diagnostic procedures (for example evaluation of the dignity of a stenosis of the iliac artery) as well as for the evaluation of the acute and long time effect of therapy of different type.
