Laktoseintoleranz: Neue Aspekte eines alten Problems

Zusammenfassung

Im klinischen Alltag ist die Laktoseintoleranz mit ihren führenden klinischen Symptomen wie abdominellen Beschwerden, Durchfällen, Meteorismus und Flatulenz nach dem Genuss laktosehaltiger Nahrungsmittel ein häufiges Problem. Es werden die primäre und die sekundäre Laktoseintoleranz unterschieden. Die Hauptform der Laktoseintoleranz, die primäre adulte Laktoseintoleranz, betrifft 75 % der Weltbevölkerung mit regional deutlich unterschiedlicher Prävalenz. Während bei bis zu 90 % der afrikanischen und asiatischen Bevölkerung eine Laktoseintoleranz besteht, sind 15 - 20 % der Deutschen und nur 2 % der Nordeuropäer laktoseintolerant. Die Laktoseintoleranz beruht auf einer mit dem Lebensalter abnehmenden Synthese des Enzyms Laktase im jejunalen Dünndarmepithel, das Laktose in Glukose und Galaktose spaltet. Die Beobachtung, dass der primären Laktoseintoleranz definierte Polymorphismen in regulatorischen Elementen des Laktose-Gens zugrunde liegen, konnte zur Entwicklung genetischer Tests eingesetzt werden. Auf diese Weise steht nun für die Diagnostik der Laktoseintoleranz ein dem H₂-Laktose-Atemtest vergleichbar sensitives und spezifisches Testsystem zur Verfügung. Mit Hilfe der Kombination beider Tests können Patienten mit primärer von denen mit sekundärer Laktoseintoleranz, bei denen die Synthese der Laktase im Rahmen anderer Erkrankungen des Jejunums vermindert ist, unterschieden werden. Unabhängig von der Form der Laktoseintoleranz basiert ihre Therapie im Wesentlichen auf diätetischen Maßnahmen. In Abhängigkeit von der Restaktivität der Laktase sollten sich die Patienten lebenslang laktosefrei oder laktosearm ernähren. Bei Patienten mit sekundärer Laktoseintoleranz wird nach Abheilung der erkrankten Darmmukosa häufig wieder eine Verträglichkeit für laktosehaltige Nahrungsmittel beobachtet.

Summary

Lactose intolerance represents a common problem in clinical practice, characterized by abdominal pain, diarrhea, bloating, and flatulence after consumption of lactose. Primary and secondary hypolactasia are differentiated. Of note, primary lactose intolerance (adult-type hypolactasia) affects approximately 75 % of the population worldwide with characteristic regional differences in prevalence, ranging from up to 90 % in Africa and Asia to 15 - 20 % in Germany and 2 % in Northern Europe. Adult-type hypolactasia is due to an age-dependent decrease of lactase synthesis in brush borders cells of the jejunum, resulting in impaired cleavage of lactose into glucose and galactose. Recently an association between adult-type hypolactasia and defined polymorphisms within regulatory elements upstream from the lactase gene has been identified. Besides the well-established H₂-breath test, genotyping for these mutations provides a reliable tool in the diagnosis of lactose intolerance. Specificity and sensitivity of the genotyping are comparable to the H₂-breath test, respectively, and the combination of both tests discriminates primary from secondary lactose intolerance. Therapy of adult-type hypolactasia is based on lifelong lactose free diet, whereas patients with secondary lactose intolerance might be able to digest lactose again after reconstitution of intestinal mucosa.
