Zusammenfassung

Studienziel: Für die Rekonstruktion ausgedehnter azetabulärer Knochendefekte in der Hüftrevisionsendoprothetik stehen unterschiedliche Implantate und Techniken zur Verfügung. Neben der prinzipiellen Eignung zur operativen Versorgung müssen darüber hinaus die spezifischen Indikationen neuer Implantate bestimmt werden. Ziel der vorliegenden Arbeit war es daher, anhand der mittelfristigen Ergebnisse nach Implantation der Sockelpfanne die Risikofaktoren für das Auftreten von Migration und Lockerung zu bestimmen und daraus das Indikationsspektrum der Sockelpfanne abzuleiten. Methode: In einer retrospektiven Studie wurden die ersten 50 der in unserer Klinik durchgeführten Sockelpfannenimplantationen bei hochgradigen azetabulären Defekten (Paprosky-Klassifikation II und III) nach einem Zeitraum von durchschnittlich 26 Monaten nachuntersucht. Unterschiede zwischen radiologisch gesichert gelockerten und knöchern integrierten Implantaten hinsichtlich der präoperativen knöchernen Situation (Defektart und ‐größe, Knochendichte) und der Implantatpositionierung wurden als Risikofaktoren eines Implantatversagens bestimmt. Ergebnisse: Intraoperativ traten im Rahmen der Lernkurve 3 implantatassoziierte Komplikationen auf. Frühpostoperativ kam es bei 10 Hüften zu einer Luxation (20 %), 2 septische Lockerungen (4 %) und 6 aseptische Lockerungen (12 %) wurden erfasst. In weiteren 7 Fällen trat vor der stabilen Integration in den ersten Monaten nach der Implantation eine signifikante Migration auf. Als Risikofaktoren für eine Lockerung wurden die fehlende Rekonstruktion des Defektes hinter der Sockelpfanne mit allogenem Knochen, eine Lateralisation des Drehzentrums (p = 0,023), das Fehlen der kraniolateralen Wand (p = 0,002) und das Vorliegen einer Osteoporose (p = 0,026) bestimmt. Schlussfolgerung: Mit der Sockelpfanne ist ein Implantat verfügbar, das bei vielen hochgradigen azetabulären Defektsituationen mit guten mittelfristigen Ergebnissen einsetzbar ist, wobei jedoch Kontraindikationen bestehen. Bei einer signifikanten Osteoporose, einer fehlenden Sklerose im Abstützungsbereich des Sockels sowie einer ausgedehnten Schädigung des lateralen Iliums bzw. bei dessen Fehlen sollten alternative Verfahren gewählt werden.
