Zusammenfassung

Studienziel: Diese prospektive Untersuchung stellt die klinischen Ergebnisse nach Defektdeckung von femoropatellaren Knorpelschäden mit einem dreidimensionalen Kollagengel (CaReS®) vor. Methode: Bei insgesamt 14 Patienten (13 retropatellar, 1 trochlear) wurden isolierte femoropatellare Knorpelschäden durch die Autologe Chondrozyten Transplantation mittels kollagenem Zellträger behandelt. In die Studie wurden ausschließlich Patienten aufgenommen, die die Einschlusskriterien der Arbeitsgemeinschaft „Geweberegeneration und Gewebeersatz“ der DGOOC und DGU erfüllten. Grundlage der klinischen Auswertung bildete das von der International Cartilage Repair Society (ICRS) entwickelte Bewertungsschema und der Brittberg-Score. Dargestellt werden die Ergebnisse bis zu zwei Jahren nach der Operation. Ergebnisse: Die ICRS-IKDC-Scores und der Brittberg-Score zeigten im Beobachtungszeitraum eine statistisch signifikante Verbesserung. Die Einschätzung des objektiven IKDC und der ICRS-Funktionsscore verbesserten sich von präoperativ 0/14 bzw. 2/14 (14,3 %) auf 11/14 (78,6 %) in den Kategorien A/B bzw. I/II nach zwei Jahren. 11 (78,6 %) der Patienten beurteilten das Ergebnis nach zwei Jahren mit sehr gut/gut. Der subjektive IKDC-Score verbesserte sich von präoperativ 32,4 ± 8,4 auf 67,8 ± 27,4 nach zwei Jahren. Schlussfolgerung: Dreidimensionale Trägerstrukturen können eine vielversprechende Weiterentwicklung der ACT zur Behandlung von femoropatellaren Knorpelschäden bedeuten. Langzeituntersuchungen müssen die in dieser Studie gewonnenen positiven Ergebnisse überprüfen.
