Summary
In endonasal endoscopic dacryocystorhinostomy bone resection to expose the saccus
lacrimalis is performed using a drill in most cases. Sometimes this impairs the endoscopic
view due to blood and rinsing liquid. Also, the rotating drill might injure surrounding
structures. Using chisels or osteotomes during endoscopic surgery is difficult, needs
assistance and permits no precise working because of the one-handed technique. As
an excellent alternative we have been using Kerrison forceps of varying sizes exclusively
for the resection of the bone above the saccus lacrimalis for the past six years.
The bone is mostly punched away, partly fractured and then removed as bone lamella.
In 134 operations that we performed this way, we could expose the saccus sufficiently
without any complications. This method proved to be so efficient in our experience,
that we can advocate it unreservedly.
