Nicht länger nur eine Erkrankung des Erwachsenenalters - Epidemiologie des Diabetes mellitus Typ 2 bei Kindern

Noch vor noch nicht allzu langer Zeit wurde der Diabetes mellitus Typ 2 (T2DM) als Erkrankung des Erwachsenenalters angesehen. Die letzten Jahre zeigten aber weltweit einen Anstieg der Manifestation des T2DM bereits im Jugendalter, parallel zur Zunahme der Adipositas. Auch in Deutschland steigt die Prävalenz des Typ-2-Diabetes bei Kindern und Jugendlichen (2,36 pro 100000) allerdings immer noch weit geringer als in den USA (Prävalenz 1-2:1000 bei Kaukasiern bis zu 50:1000 bei Pima-Indianern). Die Manifestation des T2DM erfolgt ab der Pubertät und ist meist oligosymptomatisch, sodass ein Screening in Risikopopulationen erforderlich ist. Risikofaktoren sind extreme Adipositas, Verwandte mit T2DM, Zeichen der Insulinresistenz (Acanthosis nigricans, Dyslipidämie, arterielle Hypertonie), sowie die Herkunft (Hispanier, Afro-Amerikaner, Indianer und Asiaten).
