Kontrastmittel- und B-Bild-Ultraschall in der Diagnostik und Therapiekontrolle der hepatischen Echinokokkose bei einer türkischen Patientin

Zusammenfassung

Anamnese und klinischer Befund: Eine 19-jährige türkische Patientin stellte sich wegen starker, stechender Schmerzen im rechten Oberbauch- und Flankenbereich vor. Intermittierend litt die Patientin seit einer Woche unter ähnlichen abdominellen Beschwerden.

Untersuchungen: Die Oberbauch-Sonographie zeigte eine polyzystische Raumforderung im Lebersegment VII, die eine angedeutete Kapsel aufwies. In der kontrastverstärkten Sonographie ließ sich eine randständige Hyperämie nachweisen, jedoch ohne Kontrastmittelaufnahme ins Zysteninnere. Laborchemisch fiel eine geringgradige Leukozytose von 10,46 G/l auf (Norm: 4,0 - 9,0 G/l).

Therapie und Verlauf: Die Verdachtsdiagnose einer zystischen Echinokokkose wurde serologisch bestätigt. Die Therapie der Patientin erfolgte zunächst über 8 Wochen medikamentös mit Albendazol (2 × 400 mg/d per os). Therapiebegleitend wurden Ultraschallverlaufskontrollen durchgeführt, die ein partielles Kollabieren der Zysten-Membranen zeigten. Anschließend wurde in offener Laparotomie eine 130 g schwere und 8 cm große Echinokokkuszyste durch Perizystektomie entfernt. Der postoperative Verlauf gestaltete sich komplikationslos. Postoperativ wurde die Behandlung mit Albendazol bei guter Verträglichkeit für 6 Monate fortgeführt, die Verlaufsserologie nach 10 Monaten blieb negativ.

Folgerung: Ultraschall eignet sich bei der hepatischen Echinokokkose als Screeningmethode und zur Verlaufskontrolle. Die Echinokokkus-Serologie ist nur begrenzt aussagekräftig. Vor jeglicher Punktion eines Echinokokkose-verdächtigen Herdes ist eine parasitostatische Therapie mit Albendazol über mindestens 3 Tage durchzuführen. Die Therapie der zystischen Echinokokkose ist abhängig vom Zysten-Stadium.

Summary

History: A 19-year-old Turkish woman was admitted to the emergency room because of the worsening recurrent pain in the right abdomen radiating to the right shoulder and right flank.

Investigations: Ultrasound examination showed a polycystic mass in the right lobe of the liver surrounded by a capsule. A contrast-enhanced ultrasound examination was done. It revealed, starting in the arterial phase, marginal enhancement near the capsule of the liver lesion.

Treatment and course: The suspected diagnosis of a hydatid liver disease caused by Echinococcus granulosus was confirmed by serology. The patient was treated with oral albendazole (400 mg, twice daily) for eight weeks. Follow-up ultrasound examinations showed significantly reduced „collapsing” cyst membranes. A hydatid cyst weighing 130 g and 8 cm in diameter was resected by open laparatomy nine weeks after diagnosis. Albendazole treatment was continued for six months and well tolerated by the patient. One year after surgical treatment the serology was negative.

Conclusions: Ultrasound examination is the method of choice for the initial diagnosis and follow-up of hepatic echinococcosis. Serology is not always reliable. The treatment depends on the WHO stage of the disease and individual factors.
