Innovative, transapikale Aortenklappenimplantation

Einleitung

Der operative Aortenklappenersatz mit mechanischen oder biologischen Prothesen ist die Behandlung der Wahl bei symptomatischer Aortenklappenstenose. Die Operation wird über eine vollständige mediane oder partielle Sternotomie im Herzstillstand unter Verwendung der Herz-Lungen-Maschine durchgeführt. Bestrebungen, weniger invasive Verfahren zu entwickeln, begründen sich darauf, dass die Patienten immer älter werden, schwere Begleiterkrankungen haben oder bereits am Herzen operiert wurden, sodass das Risiko einer Operation mit Herz-Lungen-Maschine nicht vertretbar erscheint.

Seit 2002 wird eine Methode der perkutanen Klappenimplantation über die Leistengefäße bei kritisch kranken Patienten mit hochgradiger Aortenstenose klinisch erprobt. Die Grenzen dieser Methode sind kleinkalibrige Femoralarterien sowie atherosklerotische Veränderungen der abdominellen und thorakalen Aorta, die bei diesen Patienten häufiger vorkommen und eine Kontraindikation darstellen.

Somit ist die in diesem Artikel vorgestellte minimalinvasive, transapikale Klappenimplantation ohne Verwendung der Herz-Lungen-Maschine eine attraktive Alternative für Patienten, bei denen weder ein konventioneller Aortenklappenersatz noch eine perkutane Klappenimplantation durchführbar ist.
