Thromboseprophylaxe in der Gynäkologie: Doppelblind-Vergleich zwischen niedermolekularem (LMWH) und unfraktioniertem (UFH) Heparin

Zusammenfassung

In einer prospektiven randomisierten Doppelblindstudie an 300 Patientinnen über 40 Jahre und mit operativen Eingriffen, die länger als 40 Minuten dauerten, wurde die Effektivität einer Thromboseprophylaxe mit unfraktioniertem Heparin (UFH) bzw. niedermolekularem Heparin (LMWH) überprüft. Wir fanden eine Thrombosefrequenz von 1,3 % in der LMWH-Gruppe und von 4 % in der UFH-Gruppe. Die Blutungskomplikationen waren in der LMWH-Gruppe ebenfalls geringer. Die Wundhämatome wurden mit 12,6% (LMWH) bzw. 18 % (UFH) angegeben. Die Zahl der Bluttransfusionen waren mit 21,3% (LMWH) und 24% (UFH) nicht unterschiedlich in beiden Gruppen. Ähnlich lagen die Verhältnisse bei dem Merkmal exzessiv hoher Blutverlust, u.z. mit 7,3% (LMWH) und 8% (UFH). Diese günstigen Ergebnisse bei nur einmaliger Applikation von 1500 aPTT-Einheiten niedermolekularen Heparins ist auf die hohe Anti-Xa-Aktivität des LMWH (2 : 1) im Vergleich zum UFH zu interpretieren. Die antikoagulatorische Wirksamkeit zeigte dagegen keine Unterschiede. Diese Vorteile gegenüber der dreimaligen Applikation weisen die einmalige Injektion von niedermolekularem Heparin ohne Dihydergot als ausreichend zur Thromboseprophylaxe in der Gynäkologie aus.

Abstract

The safety and efficacy of low molecular heparin (Sandoz AG, Nürnberg), given 1500 aPTT once daily (and tvvo placebo injections). in preventing postoperative venous thromboembolism, was assessed against sodium heparin at a dose of 5000 IU three times daily, in a unicenter doubleblind randomized study. 300 patients, scheduled for major gynecological surgery, were included in this study. Two patients (1.3%) of 150 patients developed deep vein thrombosis in the LMWH-group, the corresponding figure for sodium heparin was 6 patients (4.0 %). There was no statistically significant difference between the two groups in respect to the bleeding variables such as postoperative drainage, blood transfusions and haematoma. The antithrombotic effects were assessed with the anti-Xa assay and LMWH-fractions have shown a high antifactor Xa/activated partial thromboplastin time (aPTT) specific activity (2:1) compared to unfractionated heparin (1:1). It is concluded that one single daily injection of LMWH (without dihydergot) provides a convenient safe and effective Prophylaxis against thromboembolism in gynecological surgery.
