Fragmentation von Intrauterinpessaren als Komplikation bei der Extraktion

Zusammenfassung

Anhand von drei Kasuistiken wird über die Möglichkeit eines Abbruches der Seitenschenkel von Intrauterinpessaren berichtet. Die Fragmentation wurde bei Spontanausstoßung und bei Extraktionen beobachtet. Die abgebrochenen Teile der Pessare konnten weder sonographisch, röntgenologisch noch mittels eines Computertomogramms lokalisiert werden. In zwei von drei Fällen war eine hysteroskopische Diagnosesicherung nicht möglich, und nur einmal gelang es, per abrasionem die Pessarreste zu entfernen. Die Problematik, die sich aus der Inkarzeration und partiellen Perforation ergibt, und das empfehlenswerte therapeutische Vorgehen werden diskutiert. Die hysteroskopische Diagnostik und Lokalisation mit anschließender Extraktion durch eine spezielle Faßzange werden empfohlen.

Abstract

Three cases are reported in which the lateral arm of the intra-uterine contraceptive device had broken off. The fragmentation of the intrauterine contraceptive device was observed during spontaneous expulsion and during extraction. The broken off portions of the intra-uterine devices were not located by sonography, radiography or computer tomography. In two of three cases the diagnosis was not made by hysteroscopy. In one case the remaining parts of the intra-uterine device were removed by curettage. The problems from partial perforation and embedding of the fragments are discussed and the management described. A hysteroscopic diagnosis and extraction of the fragments is recommended.
