Erstdiagnose Epilepsie – Wie ist das medikamentöse Vorgehen?

Lernziele
Die Planung und Gestaltung einer medikamentösen Therapie nach der Diagnose einer Epilepsie öffnet vielfältige Optionen, die sich aus der relativen Vielfalt der verfügbaren Antiepileptika (AED) ergibt. Dargestellt werden Argumente zum Therapiebeginn (erhöhtes Rezidivrisiko) sowie die Auswahl der AED, die sich am Epilepsietyp orientiert (idiopathische Epilepsien mit generalisierten Anfällen versus symptomatische oder kryptogene Epilepsien mit fokalen und sekundär generalisierten Anfällen) und mögliche negative Effekte der AED auf kognitive, endokrine oder psychische Funktionen sowie die Teratogenität berücksichtigt. Damit eröffnet sich die Chance mit einem der ersten beiden AED in Monotherapie eine individuell günstige Therapie zu etablieren, die dauerhaft eine möglichst optimale Behandlung des Patienten ermöglicht.
