Abstract
The restriction of postoperative flexion after MCP-Silastic implantation of spacers at the average of 50 degress leads to a limitation for indications for such procedures. This study was performed to demonstrate the influence of physiotherapy in the postoperative period by reproducible measurement.
After designing a new ,,digital-ergometer" a comparative study was performed. We could examine 34 patients 103 MCP-Silastic implants after average follow-up time of 4.8 years. Active isometric flexibility, isometric and isotonic torque moments were measured at MCP-level. As controls healthy test-persons and patients with natural course of RA were examined.
The results of a group A with consecutive physiotherapy during the postoperative period were superior in 20% to those of a group B (max. 8 weeks therapy) in terms of flexivility of MCP joints. The torque moments at MCP joints could be increased in up to 35 % by long-term physiotherapy. The results encouraged the authors to develop a training program for postoperative therapy.
Zusammenfassung
Die Einschränkung der postoperativen Beugefähigkeit nach Implantation von Silastic-Implantaten an Fingergrundgelenken von im Mittel 50 Grad engt die Indikationsstellung für derartige Eingriffe ein.
Nach Konstruktion eines speziellen ,,Fingergelenk- Ergometers" konnten im Mittel von 4,8 Jahren postoperationem 34 Patienten mit insgesamt 103 MCP-Implantaten bezüglich der Beugefähigkeit, der isometrischen und isotonischen Drehmomente am Fingergrundgelenk verglichen werden. Als Kontrollgruppen waren gesunde und cP-Patienten untersucht worden.
In der Gegenüberstellung einer Gruppe A mit langfristigen Übungsbehandlungen war diese der Gruppe B (maximal 8 Wochen Therapie) hinsichtlich der Beweglichkeit bzw. Beugefähigkeit um 20% überlegen. Ebenso war das Drehmoment bzw. die resultierende Kraft am Fingergrundgelenk um ca. 35% durch langfristige Übungen im Vergleich gesteigert. Diese Ergebnisse ermutigten uns, ein Übungsprogramm für die postoperative Therapie zu entwickeln.
