Die endoprothetische Versorgung des oberen Sprunggelenkes bei rheumatoider Arthritis

Abstract

The involvement of the lower extremity in R. A. endangers the patient's walking ability.

Ergotherapeutical aspects taught us to preserve all mobile capacity because of the multiloculated joints attack of this disease. Knees and hips are more frequently affected in R. A. but additional stiffness in ankle joint will lead to a bad walking capacity and a wooden walk. Each local loss of mobility must be compensated by neighbouring joints. To preserve mobility, total joint replacement of the ankle joint is an excellent alternative to arthrodesis and orthopaedic shoe appliances.

Zusammenfassung

Die rheumatische Affektion der unteren Extremität gefährdet die Gehfähigkeit des Patienten. Ergotherapeutisehe Aspekte haben uns gelehrt, alle mögliche mobile Kapazität der unteren Extremitäten zu bewahren. Da häufigkeitsmäßig eher Knie- und Hüftgelenke befallen sind, kann bei zusätzlicher Einsteifung der oberen Sprunggelenke eine schlechte Gehfähigkeit das Gangbild stark alterieren. Der Gelenkflächenersatz des oberen Sprunggelenkes unter strenger Indikationsstellung bietet hier eine deutliche Verbesserung des Gangbildes sowie erhebliche Schmerzbefreiung. Die Indikation sind denen der Arthrodese vergleichbar, zusätzliche Eingriffe am unteren Sprunggelenk werden bei gleichzeitigem Befall dieser Gelenkanteile notwendig.
