Abstract

The influence of progressive cycle ergometer test on the purine metabolism of gout patients was examined. 14 male patients suffering from primary gout - 7 took allopurinol, 7 had no drug treatment - and 10 healthy male controls took part in the investigation. The maximum progressive cycle ergometer test led to no significant changes in the serum uric acid levels but to an increase of the serum levels of hypoxanthine and inosine reaching statistical significance (p < 0,01) in the group of patients without drug treatment and in the control group. In all groups the renal elimination of hypoxanthine increased with statistical significance (p < 0,01). Short term training like progressive cycle ergometer test can be done in gout patients without any hazard to provocate essential changes of purine metabolism.

Zusammenfassung

Um den Einfluß stufenweiser Ergometerbelastung auf den Purinstoffwechsel bei Gichtpatienten zu erfassen, wurden 14 Männer mit primärer Gicht (7 mit Allopurinoltherapie und 7 ohne Allopurinolbehandlung) sowie 10 gesunde Probanden untersucht. Die stufenweise Ergometerbelastung führte bei Gichtpatienten und Kontrollpersonen zu keinen signifikanten Veränderungen der Serumharnsäure, wohl aber zu Anstiegen der Vorstufen Hypoxanthin und Inosin im Serum, welche in der Gruppe der Gichtpatienten ohne Allopurinoltherapie und in der Kontrollgruppe statistische Signifikanz (p < 0,01) ereichten und nach der Belastung in allen Gruppen zu einer signifikant vermehrten renalen Elimination von Hypoxanthin (p < 0,01) führten. Kurzzeitbelastungen im Rahmen von Trainingsprogrammen können demnach bei Gichtpatienten durchgeführt werden, ohne daß im Hinblick auf eine Provokation eines Gichtanfalles bedeutsame Veränderungen des Purinstoffwechsels befürchtet werden müssen.
