Aktueller Stand und kritische Wertung der Behandlung mit Auranofin, parenteralem Gold und D-Penicillamin

Abstract

The article reviews the results of controlled comparative studies with Auranofin and intramuscular gold or D-Penicillamin, respectively. Particular emphasis is placed on discussing the profile of action, the rates of remission, slowing-down of destruction by x-rays, the profile of side effects, the numbers of withdrawals, and the necessary laboratory controls. On the whole, Auranofin appears to be less toxic, the efficacy being obviously the same. Questions which are still unsolved are, inter alia, the problem of compliance, the switching-over from intramuscular to oral gold therapy, and the possible combination with antimalarial drugs.

Zusammenfassung

Es wird eine Übersicht über die Ergebnisse kontrollierter Vergleichsstudien zwischen Auranofin und intramuskulärem Gold bzw. D-Penicillamin gegeben. Dabei werden besonders das Wirkungsprofil, die Remissionsraten, die Verlangsamung der Röntgendestruktion, das Nebenwirkungsprofil, die Absetzquoten und die notwendigen Laborkontrollen diskutiert. Bei offensichtlich vergleichbarer Wirksamkeit scheint Auranofin insgesamt weniger toxisch. Offene Fragen betreffen die Compliance, das Umsetzen von der intramuskulären auf die orale Goldtherapie und die mögliche Kombination mit Antimalariamitteln.
