Zusammenfassung
Es wird das computertomographische Bild des seltenen inflammatorischen Bauchaortenaneurysmas anhand 4 eigener Fälle vorgestellt. Die Erkrankung, deren Ätiologie nicht definitiv geklärt ist, befällt überwiegend Männer im 5., 6. und 7. Lebensjahrzehnt. Computertomographisch zeigen sich neben den typischen Veränderungen eines Aortenaneurysmas hufeisenartige, ringartige oder tumorartige homogene perianeurysmatische Massen mit intensivem Dichteanstieg nach i.v. Kontrastmittelgabe. Zur differentialdiagnostischen Abgrenzung dieser periaortalen Weichteilformation von einer Dissektion und retroperitonealen Lymphomen ist die i.v. Applikation des Kontrastmittels in Bolusform erforderlich. Durch das unterschiedliche Enhancement von Aortenlumen, Fibröse und Lymphomen ist eine Unterscheidung dieser Strukturen sicher möglich.
Summary
The computer tomographic appearances of four patients with the rare inflammatory aneurysm of the abdominal aorta are described. The condition, whose aetiology is not known, affects mostly men in the fifth, sixth and seventh decade. Computed tomography shows, in addition to the typical changes due to an aortic aneurysm, horse-shoe shaped, circular, homogeneous perianeurysmal masses, which enhance after intravenous contrast. Bolus injection of contrast is necessary in order to differentiate these from a dissection or retroperitoneal lymphoma. Distinction between these conditions is possible as a result of the various types of enhancement which occur in the aortic lumen, in fibrosis and in lymphoma.
